Industrial Waste Forms

There are three types of forms available below. PDF forms require the free Adobe Acrobat Reader.

Special Discharge Forms are for waste that have special discharge requirements and/or special reporting requirements. Special discharge wastes include groundwater, surface water, landfill leachate, landfill condensate, and industrial waste hauled by a MCES-permitted liquid waste hauler.
Liquid Waste Hauler Forms are used by MCES-permitted liquid waste haulers for reporting the waste that they transport and discharge into the Metropolitan Disposal System.
Standard Discharge Forms are for waste that does not fall into one of the above types (i.e., Special Discharge or Liquid Waste Hauler). These forms are for industrial waste discharged into the Metropolitan Disposal System using an on-site sanitary sewer connection.
