SAC Determination Forms

 

  • SAC Determination Application Transmittal (Transmittal-A) (PDF)
    A required form, which includes the submittal checklist, to be sent for all determination requests for Metropolitan Council.
  • Additional Submittal Requirements (Transmittal-B) (PDF)
    A list of additional submittal questions and information needed in order to complete the SAC determination for certain business uses, this form must be filled out and signed by the business owner.
  • Affidavit of Business Use–Food/Drink Establishments (Affidavit-A) (PDF)
    This form is required to be submitted with the other items listed on the SAC Determination Application Transmittal if the business use is a food/drink establishment. This affidavit must be filled out and signed by the business owner.
  • Affidavit of Business Use–Manufacturing (Affidavit-B) (PDF)
    This form is required to be submitted with the other items listed on the SAC Determination Application Transmittal if the business use is a manufacturing establishment. This affidavit must be filled out and signed by the business owner.
  • Affidavit of Vacancy (Affidavit-C) (PDF)
    This form is required when requested to validate the vacancy of a property. This form must be filled out and signed by a Local Government Unit Official.
  • Affidavit of Property Use (Affidavit-D) (PDF)
    This form is required when requested to validate the use of a property prior to the Look-Back Period when no SAC history or documentation exists. This form must be filled out and signed by the Local Government Unit Building Official and the Building Official's manager.
  • Reclaim–Conveyor (Reclaim-C) (PDF)
    A submittal checklist for reclaim conveyor car wash systems, all of the items on this checklist must be submitted with the SAC Determination Application Transmittal.
  • Reclaim–Rollover (Reclaim-R) (PDF)
    A submittal checklist for reclaim rollover car wash systems, all of the items on this checklist must be submitted with the SAC Determination Application Transmittal.
  • Phased Development Plan Checklist (Phased Development-A) (PDF)
    This form is a checklist of required items to be considered for a Phased Development Request by the Local Government Unit. This form must be signed by a Local Government Unit Official and submitted with all items on this checklist.
